Forms: Email Sign up to Klaviyo

Setup is required before this template will work. Follow the steps below.

Mesa allows you to create Forms that can be hosted within your Shopify store to collect customer data. In this example, Mesa Forms will add the contact information collected from the customer to subscribe to an existing Klaviyo list of your choice.

Follow the steps below to get this set up for your store.

1. In the Mesa dashboard, click Create New Workflow.

2. Add Email Sign up to Klaviyo in the Name field.

3. Click Continue.

4. On the first step, search and select the Form by Mesa app in the Apps' menu. The Form by Mesa step is used when you want to create a form for your customers to submit.

You can build a form from scratch, or copy the HTML code from our Form Builder, modify it, and use it on any page on the web. More details can be found on Mesa Forms: Click Here. You will want your form to look like the following image.

Under the Advanced Options, add a Success Redirect URL that will allow you to redirect customers to a specific link or to embedded pages on your Shopify store. You can also enable a Captcha to protect from spam.

5. Next, paste the Form Embed Code onto a page on your store which requires the customer to be logged in and also has order details. 

This will allow the Form Embed Code to capture the Customer ID and Order ID as hidden form fields. You can click here for details on how to add the Form onto your Customer Order Status Page. 

6. On the second step, search and select Klaviyo in the App's menu. Then click Use this on the List Subscribe action.

7. Set up your credential to connect your Shopify to Klaviyo. You can learn more about creating a credential for Klaviyo: Click Here.

In the App Configuration section, you can select the Klaviyo List that you would like the Form information added. The following screenshot uses the example of the email shared by the customer. 

8. The rest of the information in the Data Mapping step will be unique based on the information that you are collecting from the Form. You can utilize Mesa's Tokens feature to find the right Token for the customer's email address. This lets you map out the data to send to Klaviyo. In this example, the values we’ve added are Form Name, Form Email, and Form Phone

9. Once the setup is complete, hit the Save button at the top of the workflow to save your changes.

10. Hit the Enable button when you are ready to start using the workflow. Now, you're all set! 🎉
