Add a Mesa Form to your Order Status page

Mesa Forms give you the flexibility to collect valuable customer information in multiple places on your store. A great place to provide a form is on the Shopify Order Status page after a customer completes checkout.

Follow the series of steps below to add your Mesa Form to the Order Status page:

1. In a Mesa workflow where you've created a form, click 'Open Form Builder' in the Form trigger step and select the Get Code tab in the Form Editor:

2. Delete the code at the top that appears immediately before and after the code that reads:

You should only be removing the comment code:

3. Next, click the Copy all code button to copy the form code.

4. Go to your Checkout Settings (Settings in Shopify admin --> Checkout) and paste the copied form code in the field labeled 'Additional Scripts' in the Order processing section:

5. Save your changes.

After saving, your form will appear on the Order Status page after customers checkout. You can preview the look of the form by going to any Order on your store, click the More Actions dropdown, and then select View order status page from the dropdown menu.
